An intensive rehabilitation program to enhance recovery process for survivors of arthritis; stroke, cerebral palsy, pains and other disabilities.
This program is a community-based rehabilitation program designed to enhance healthcare delivery in various communities.
The overarching goals for the program are to:
1. Deliver healthcare services through a strong consortium, in which every consortium member organization is actively involved and engaged in the planning and delivery of services
2. Utilize and/or adapt an evidence-based or promising practice model(s) in the delivery of healthcare services.
3. Improve population health, and demonstrate health outcomes and sustainability.
4. Expand the delivery of healthcare services to include new and enhanced services exclusively in rural and urban communities
A program organized by Road 2 Recovery that is committed to provide affordable, high-quality health care coverage to survivors, no matter their age or life circumstance. We stand by survivors during recovery process and we offer them an amazing array of healthy discounts, rehabilitation services, behavioral health services and special health programs.
Please note that one sponsorship order covers 200 patients.
